- New patients can create an account, apply online and print the 30-day receipt. See the New Patient page for details.
- Existing patients can view account information, renew an application, pay online, and print the 30-day receipt. See the Renewal page for details.
- Note: If you don’t want to use the online system, you can still apply or renew by mail.
